Smile Plus Membership plan is only valid for a 12 month period and will renew after 12 months.

Your effective date is the day you sign up and your renewal date is the same date every year (you will be reminded as the date gets closer).

Family members cannot be substituted in for another family members plan benefits. Additional family members can be added at any time.

Cleanings must be performed before the end coverage date. Regular cleanings are for patients with healthy gum tissue in the absence of gum disease.

Patients with periodontal (Gum) disease can purchase the Perio plan which includes 3 periodontal maintenance cleanings in their 12 month plan period. 

This is a dental discount plan and is NOT dental insurance. It cannot be combined with any other dental insurance or discount plans.

Payment is due at time of service for any work received.

Rates are subject to change annually.

Membership is valid only at Ethos Family Dental, therefore if you are referred to a specialist outside our office the benefits offered to members will not apply.

Veneers and Invisalign are considered cosmetic treatments and are not covered benefits.
